Your Privacy Matters to Us

 

At Frontier Diagnostics Guelph ("FDG"), we understand that your personal health information is highly sensitive and deserves to be handled with the utmost respect, confidentiality, and security. This Privacy Notice explains how we collect, use, disclose, safeguard, and provide access to personal health information in compliance with the Personal Health Information Protection Act, 2004 (PHIPA).

Who We Are

FDG is a diagnostic imaging provider located in Ontario. We deliver imaging services, including X-Ray, ultrasound, mammography, and bone mineral density, and collect personal health information as part of the clinical care we provide to patients.

 

What Is Personal Health Information?

Personal health information includes any recorded information about an identifiable individual, including health history, imaging results, health card number, contact information, and any other information needed to provide healthcare services. 

How We Collect, Use, and Disclose Personal Health Information

FDG collects, uses, and discloses personal health information only as necessary to provide healthcare services and to meet legal and operational obligations. We may use or disclose your information to:

  • Identify you and provide, plan, evaluate, and coordinate diagnostic imaging services and related care;

  • Assess your healthcare needs and communicate results, diagnoses, or treatment recommendations to you and your authorized healthcare providers;

  • Schedule and confirm appointments, respond to inquiries, and maintain ongoing communication with you;

  • Follow up regarding testing, treatment, care, and billing matters;

  • Submit claims to the Ontario Health Insurance Plan (OHIP), the Workplace Safety and Insurance Board (WSIB), private insurers, and others as necessary; 

  • Conduct quality assurance, risk management, education, training (on an anonymous or de-identified basis where possible), health system planning, research, statistical analysis, and service improvement activities;

  • Carry out administrative and management functions such as reporting, evaluation, and resource planning;

  • Comply with applicable laws and regulatory requirements, including responding to lawful requests, audits, court orders, or requirements of regulatory bodies such as the College of Physicians and Surgeons of Ontario or obligations under applicable legislation;

  • Fulfill any other purpose identified at or before the time of collection, or as otherwise permitted or required by law; or

  • Support healthcare research, education, or system planning in limited circumstances, typically using de identified information and only as permitted by law.

 

Disclosure of Personal Health Information

Your personal information is not disclosed to third parties without your consent, except as permitted or required by law. FDG does not sell, rent, or lease Personal Health Information.

We may share information with healthcare providers involved in your care, emergency contacts (where appropriate), employees, radiologists, and contractors to the extent required for them to provide the services you have requested or for the purposes described in this Privacy Policy. These parties are required to protect your information and use it only for authorized purposes.

Consent

Frontier Diagnostics seeks informed consent for the collection, use, and disclosure of personal information, except where inappropriate or where permitted or required by law. Consent may be expressed orally or in writing, or implied through your actions. For example, consent for the collection, use, and disclosure of personal health information related to your care (including communication with your referring provider) is generally implied when you book an appointment or attend our facility for services.

In limited circumstances permitted by law, we may collect, use, or disclose personal information without your knowledge or consent, including where:

  • You are unable to provide consent in a timely manner and it is in your best interests;

  • We are required to comply with a court order, or other legal obligation; or

  • Disclosure is otherwise authorized or required by law.

 

Your Rights and Choices

You have certain rights regarding the personal health information in our custody or control. You may contact our Privacy Officer to:

  • Request access to, and obtain a copy of, your personal health information;

  • Request correction of information that you believe is inaccurate or incomplete;

  • Withdraw or limit your consent to our collection, use, or disclosure of your personal health information (subject to legal or contractual restrictions); or

  • Be notified if your personal health information is lost, stolen, or accessed without authorization where notification is required by law.

 

Changes to This Policy

 

We may update this Privacy Notice from time to time to reflect changes in our practices or legal requirements. The most current version will be made available on our website and at our on-site facility.
