What to Bring With You

To ensure your walk-in visit is smooth, make sure you have these two things:

Your Health Card (OHIP): If your card is expired or lost, please bring an alternate form of government ID.
The Requisition Form: This is the most important part! We cannot X-ray you without a valid doctor's order.
- Tip: Did your doctor give you a form for a different clinic? We can accept it! In Ontario, you can take your requisition to any imaging centre you choose.
